Referral process for Phase IV Cardiac Rehab
Before being referred, all patients must be clinically assessed. During the assessment, if the patient meets the criteria, you can refer them onto the Phase IV Cardiac Rehab programme.
Patients can come onto the scheme either through the primary care pathway, or the secondary care pathway.
This referral method is approproate if:
- more than six months has elapsed since the patient had an acute cardiac event, or
- it has been more than six months since the patient was discharged from a CR Phase III programme, or
- the patient has CHD history, but no recent acute event
If it has been less than six months since an acute event without clinical assessment or undertaking a CR Phase III programme, you should redirect the patient via Phase III.
This referral method is for patients who have successfully completed a CR Phase III programme.
For each referral pathway, the next steps are as follows:

health professional identifies patients suitable for programme, and gets consent from patient, so we can contact them

health professional fills in our referral form in full (and if BACPR transfer form has been issued, tells the patient to bring to the assessment)

once we receive the form, we will contact the patient within seven working days to book them in for an assessment at their chosen CCC leisure centre

we accept patient onto scheme if the Phase IV Cardiac Rehab Instructor is satisfied that:

patient meets CR referral criteria
-
clinical assessment has been completed
-
there have been no new events or symptoms in the interim period since referral date

The patient will be eligible if:
- they are over 18 years old, and
- they have successfully completed a CR Phase III programme, or
- six months have elapsed since acute cardiac event/discharge from Phase III, or
- there is CHD history but no recent acute event in the last six months
They also need to have at least one of the following:
- post-myocardial infarction
- post-coronary artery bypass graft surgery
- post percutaneous coronary intervention (with or without prior cardiac event)
- stable angina
- stable chronic heart failure
- post valve replacements
- a permanent pacemaker
- an implanted cardioverter defibrillator
The patient will not be eligible if they have:
- complicated myocardial infarction
- unstable angina
- uncontrolled ventricular dysrhythmia
- uncontrolled atrial dysrhythmia compromising cardiac function
- third degree heart block
- aortic stenosis
- aneurysm, including ventricular aneurysm
- acute illness
- thrombophlebitis
- thrombo-embolism
- acute congestive heart failure
- significant emotional distress
- pericarditis and myocarditis
- recent ECG changes suggesting other acute cardiac events
- resting systolic blood pressure of more than 180mmHg and/or resting diastolic blood pressure of more than 100mmHg
- any uncontrolled health conditions
